Solution

First, lets set the app to count in miles and show two decimal places:

If $4.0 initial amount includes the first 1/4 of mile (be sure to tap on “Minimum fare” button while setting up the tariff and enter 0.25 there):

I’m not sure for the charging step, I left it as 0.5 for the first tariff and 0.1 for the second one. I’m not sure in what steps do you charge. You can just set it as zero.

I also set 5 miles per hour as a threshold to start charging for the time, you might come to some over speed based on your experience.

As for 1.0 dollar for extra passenger, you can either add one dollar on a trip start as extra. Or you can have a tariff with 5.0 dollar as a minimum or entry fare for 2 passengers and 6 dollars for 3 passengers, and so on.
